Why nights are worse
Position. Lying on the shoulder compresses it. Lying on the other side often lets the top arm drop forward across the body, which also loads the joint. Lying flat removes the small supportive muscle activity that is present when you are upright.
Reduced distraction. With nothing else competing for attention, a moderate daytime ache becomes the dominant sensation.
Tissue irritability. Tendon and bursa irritation frequently produces a pattern of pain at rest and at night, distinct from pain only on movement.
The common culprits
- Rotator cuff-related shoulder pain. Usually pain with overhead reaching, reaching behind, and lying on the shoulder.
- Frozen shoulder (adhesive capsulitis). Progressive loss of motion in multiple directions, often with significant night pain, sometimes without an injury.
- Neck-referred pain. Pain felt in the shoulder and upper arm, but coming from the cervical spine. Often accompanied by neck stiffness or symptoms extending past the elbow.
Distinguishing these matters — they are managed differently.
What helps in the meantime
- Sleep on the non-painful side with a pillow supporting the top arm so it does not fall forward
- On your back, a small pillow under the elbow of the painful arm often reduces load
- Keep the shoulder moving during the day within comfortable range; complete rest tends to increase stiffness
When to be evaluated
Night shoulder pain lasting more than two to three weeks is worth an evaluation, as is any shoulder that is progressively losing motion. Frozen shoulder in particular responds better to early recognition.
Red flags
Seek prompt medical care for shoulder pain after significant trauma with deformity or inability to lift the arm, shoulder pain with chest pain or shortness of breath, or shoulder pain with fever and a hot swollen joint.
How Austin Chiropractic Care approaches it
We evaluate the shoulder and the neck together, screen for neurologic involvement, and test how the shoulder behaves under specific loads. Care may include chiropractic care, neurologic rehabilitation to restore control and range, and laser therapy for temporary relief of minor muscle and joint pain around the shoulder.
Frequently asked questions
- Why can I not sleep on that side?
- Side-lying compresses the shoulder and can reduce space in the subacromial region, which an already irritated tendon or bursa tends to register.
- Is night pain serious?
- Night pain is common with rotator cuff-related and frozen shoulder problems. Persistent night pain that does not change with position, or comes with fever or unexplained weight loss, should be medically evaluated.
