What "shooting" usually indicates
Sharp, electric, travelling pain generally points toward nerve tissue being irritated somewhere along its path. That path starts at the lumbar spine, runs through the pelvis and buttock, and continues down the back of the thigh.
But travelling leg pain is not automatically nerve pain. Joints and muscles refer pain too, and referred pain can feel deep, achy and hard to localize.
Common sources
Lumbar nerve root irritation. A disc or joint change in the low back irritates a nerve root. Symptoms often follow a strip down the leg, may include tingling or numbness, and frequently change with position — worse with sitting or bending for some people, worse with standing and walking for others.
Piriformis or deep gluteal irritation. Symptoms concentrated in the buttock, often provoked by sitting on a hard surface or by hip rotation.
Hamstring origin problems. A deep, localized ache at the sit bone, worse with sprinting or sitting, without the electric quality.
Hip joint referral. Groin and lateral thigh pain that worsens with weight bearing and rotation.
What the pattern tells you
- Below the knee, with tingling or numbness: more likely nerve root involvement.
- Buttock and back of thigh only, deep and aching: could be referred rather than nerve.
- Worse with coughing or sneezing: raises suspicion of disc involvement.
- Worse with walking, better with sitting or leaning forward: worth evaluating for a stenosis pattern, more common with age.
When to be evaluated
Any leg symptoms that persist beyond a couple of weeks, that recur, or that include numbness, tingling or weakness should be examined. An evaluation can tell you whether the nerve is genuinely involved, where the irritation is coming from, and what positions and loads to change.
Red flags
Seek urgent medical care for weakness that is getting worse, numbness in the groin or saddle area, loss of bladder or bowel control, or leg pain with fever or unexplained weight loss.
Frequently asked questions
- Is all leg pain sciatica?
- No. Referred pain from a joint, hip pathology, hamstring injury and vascular causes can all produce leg symptoms. Sciatica specifically refers to symptoms along the sciatic nerve distribution.
- Does shooting pain mean nerve damage?
- Not by itself. Nerve tissue can be irritated and sensitive without being damaged. Loss of strength or reflex changes is a more meaningful sign and should be evaluated.
